Chronic Pain Self Management
The Chronic Disease Self-Management Programs consist of 2.5 hour workshops delivered every week over a six week period. Workshops are facilitated by two trained leaders (staff or non-health professionals living with a chronic illness). Workshops are designed to disseminate information and enhance skills related to living a healthy life (healthy eating and exercise) including managing pain, fatigue, stress and medications; communicating with healthcare professionals, making decisions about treatments and planning ahead and problem solving.
The workshops are designed to give participants (patients) and their families the skills and tools to help manage the daily challenges of their chronic conditions.
TAIBU CHC works in partnership with the Central East - Community Care Access Centre (CE-CCAC) to deliver these workshops within the Malvern and the wider neighborhoods of Scarborough.
