Older Adults Program

The mission of the TAIBU’s Older Persons Health and Wellness Program is to improve the quality of life of older persons of Malvern residents with a special focus on the Black community in the Greater Toronto area. TAIBU applies proven approaches and Best Practice Guidelines in the care of Older persons addressing the level of their health needs by providing primary, secondary and tertiary health promotion and preventive health services by empowering older persons to improve and increase control over their health.

Goal

  • The goal of the program is to provide a culturally and linguistically high standard of older care that promotes and empowers self management care.
  • To empower older persons to increase control over and to improve their health
  • To support healthy aging, reduce the risk of disease and promote maintenance of function, confidence and engagement
  • To use best practice
  • To do outcome measurement
  • Engage partners within the community
  • To build sustainability

Objectives

  • To provide individualized patient care and an intensive lifestyle management care, promoting and supporting the development of self-care behaviors as well as sustaining positive lifestyle changes
  • To build clinical partnership with experts in providing the best care of older persons with complex issues and needs.
  • To build effective partnership with agencies like CCAC, Toronto Public Health, local hospitals, community health centres, churches, Caribbean Chapter of the CDA, Social Services, etc
  • To identify and train volunteer coaches to reach and bolster individuals in need of additional support in making or sustaining their needed lifestyle change.
  • To continue to recruit and train different ethnic lay educators to participate in their ethnic specific group sessions so as to culturally and linguistically communicate and enhance the interventions.
  • To establish an older person Advisory Committee to guide TAIBU and further develop a responsive program to the needs of elders.
  • To develop a Continuous Quality improvement process in order to monitor quality of care as well as to evaluate and monitor patient behavior and outcomes
